Patient Forms

To save you time during your next visit, Texas Eye and Laser Center offers our New Patient Forms online.  To access these forms, simply download the PDF file below, print it and fill it out at home, then bring it with you on the day of your appointment to either our Hurst or Ft Worth locations. Filling these out in advance of your first visit will minimize the time you spend in the waiting room filling them out when you get here.  If you're not sure which forms you need, please call 817-540-6060 for assistance or email us your questions at info@lasikdr.com.
