To save you time during your next visit, Texas Eye and Laser Center offers our New Patient Forms online. To access these forms, simply download the PDF file below, print it, fill it out at home, and bring it with you on the day of your appointment to either our Hurst or Fort Worth location. Filling these out in advance of your first visit will minimize the time you spend in the waiting room filling them out when you get here.
